RVOT STENTING IN A 13 YR OLD BOY : NH MMI Narayana Superspeciality Hospital, Raipur

Raipur //// A 13yr boy form Odisha, was evaluated at an outreach camp at Bolangir, under the congenital heart diseaseprevention and treatment programme, under RBSKY scheme. He was found to have Tetralogy of Fallot ( Large Holebetween the two lower chambers of heart and severe narrowing of the connection between right side of heart andlungs), in the screening echo cardiography at Bolangir. On re assessment at NH MMI, Raipur, it was found that he isnot suitable for complete corrective surgery immediately. However the child was sick, oxygen saturation was only60% and he was not growing well. (Wt – 20 kg as against 45kg at the age of 13 years ). Hence some thing wasneeded to increase the oxygen saturation by increasing the blood supply to lungs. Surgery like BT shunt at this agewould have been high risk and would have increase the risk of subsequent corrective surgery. Hence STENTING of

The RVOT ( the connection between heart and lungs with a stent was planned. However, this kind of procedure is avery rare procedure requiring meticulous planning and high degree of skill & team work. There is no dedicated stentavailable for this procedure, hence the stents used in other part of body ( peripheral stent) was used . The procedurewas done on 25.11.21 at our hospital. It was done by Dr. Sumanta Shekhar Padhi ( Senior interventional cardiologist)and Dr Kinjal Bakshi ( Senior Pediatric Cardiologist ). It was done under conscious sedation anaesthesia -given by DrRakesh Chand / Dr Nilima Das. Immediately after the procedure the oxygen saturation increased from 60 % to about90%.He could be discharged from the hospital 48 hr after the procedure. This was the 2nd such procedure done in the state. The 1st such was also done in our hospital by the same teamabout 9yr back. It was the 1st such case performed in a child from Odisha.
